Families used to think nursing homes, assisted living communities and other centers were the only solutions for their loved ones when they began to change physically and cognitively. Now seniors, disabled individuals, and their families have a right to stay home. Our home care program is designed to keep our clients happier, healthier, and at home.
As you might know “Home Care” is a simple phrase that encompasses a wide range of health care services. These services are delivered at home to recovering, disabled, chronic and terminally ill individuals in need of assistance with essential activities of daily living needs.
Home Care is appropriate whenever a person prefers to stay in the comfort of their own home but still needs ongoing care that cannot easily or effectively be provided by family or friends. Elderly and disabled individuals are electing to receive home care services as their physical capabilities diminish.
Today, people are electing to live at home independently instead of being institutionalized.
Adults who are disabled and recuperating from acute illnesses are choosing Home Care whenever possible.
Infants and children who are chronically ill and disabled are receiving personalized treatment in their familiar and secure home environment. Home care in addition to Hospice services enables terminally ill patients to receive compassionate care and maintain dignity until the end of their life, while at home.
It is ultimately CDE Health Care Services’ goal to keep individuals engaged in life mentally, emotionally, physically, and socially. This can make a difference in their well being and promote happier, healthier lives while at home.

What is Medicaid?
Medicaid is a state administered health program for eligible individuals and families with low incomes and resources that are jointly funded by both the state and federal government through The Center for Medicare & Medicaid Services. It offers coverage for necessary inpatient and outpatient medical care, nursing facility services for children under age 21, rehabilitation and physical therapy services, and home and community-based care to certain persons with chronic conditions.

How to qualify for In-home care services through Medicaid/Missouri Healthnet Program.
To qualify for our services through Title 19, you must be eligible for Medicaid/Missouri Healthnet. If you do not have Mo Healthnet/Medicaid coverage and are in need of in-home health care services, our agency will personally come out and process a MO Healthnet/Medicaid application for you at no charge!
Our provider representatives will provide you with all the assistance you need.
Our nurse will schedule a preliminary assessment visit with you. If it is determined that you meet the Level of Care requirements, your referral will be forwarded to the Missouri Department of Health and Senior Services or the appropriate state contracted facility for qualifications of these services. This is a state funded program which means that all recommendations are made by the Department of Health and Senior Services or the appropriate state contracted facility. They will review your assessment and re-determine that the level of care requirements were met which will result in:

The Medicaid Program ServicesHomemaker Chore (HC):
Homemaker chore services consist of general household cleaning provided by a trained homemaker. These tasks are necessary for a clean, safe and healthy environment. Such chores are: laundry, ironing, dusting, mopping, vacuuming, disinfecting, polishing, medical/essential errands to grocery shop, pay bills, pharmacy and post office.
Personal Care (PC):
Personal care services are maintenance services to assist individuals with activities of daily living such as bathing, dressing, grooming and assistance with self administered medication. Assistance with toileting needs for individuals with bowel and bladder incontinence and, medical related household tasks.
Advanced Personal Care (APC):
Advanced personal care services are maintenance services to assist individuals with activities of daily living when such assistance requires devices or procedures related to altered body functions (skin integrity, gastrostomy tubes, catheter care, ostomy care, assisting with lifting and transferring of a client, passive range of motion).
Professional Non-Skilled Nurse Visits
Registered and Licensed Practical Nurses will provide services such as:
All clients must have 2 General Health Assessments provided by a professional nurse 2 times annually.
Respite Care and Advanced Respite Care
Respite care services are maintenance services of companionship and home activities to provide temporary relief for the regular caregiver of a dependent adult. Advanced respite care offers relief to a caregiver who lives with a client having specials needs (Bed fasting requires turning clients every 2 hours, behavioral disorders, prompting for self-administered medication, special monitors due to swallows, Parkinson’s, ALS and strokes).
Healthy Children and Youth Program (HCY)
This is a comprehensive, primary and preventive health care program for Medicaid eligible children and youth until the age of 21. This allows children to remain in their home as a "healthy child" or "healthy youth.”

Private Pay Service Individuals using Long-Term Care Insurance or personal funds are considered ‘private pay’ since they are not being covered by a government program. Insurance plans often have restrictions and limits. If your insurance does not cover the services you need, or discontinues payment for services already received, you are generally left to pay the difference ‘out-of-pocket.’
Paying privately for home healthcare services is often an option for individuals who remain living independently in their homes but are not qualified for state or federal government funded programs.
For those individuals who are interested in our out of pocket program, we offer reasonable/competitive rates.
